IMPORTANT INFORMATION FOR PALMS WEST EMPLOYEES

THIS WEBSITE IS STRICTLY FOR PALMS WEST'S UNIFORM CONVERSION. IT IS NOT DESIGNED FOR ONLINE PURCHASES AT THIS TIME. YOU CAN PRE-SELECT AS MUCH AS YOU WANT AND IF THE ITEMS YOU SELECT EXCEED YOUR VOUCHER AMOUNT YOU WILL BE OBLIGATED TO PAY THE DIFFERENCE AT THE TIME OF PICKUP (PAYROLL, CREDIT CARDS AND CASH WILL BE ACCEPTED).

VOUCHERS HAVE TO BE PRESENTED AT THE TIME OF PICKUP AT EITHER A ONSITE HOSPITAL SALE OR AT ONE OF MEDWEAR UNIFORMS RETAIL STORES.YOU WILL BE NOTIFIED WHEN UNIFORMS ARE READY BY EMAIL OR PHONE.

ONSITE HOSPITAL SALES WILL HAVE MERCHANDISE AVAILABLE FOR PURCHASE.

_1. Uniforms that have a RED ASTERISK * are considered the preferred (not required) hospital uniforms. You may only use your
___voucher to purchase Cherokee or Dickies uniforms.

_2. Uniforms without a RED ASTERISK *are still approved, however, employees will be responsible for any additional costs above the
___voucher approved amount. (Payroll Deduction, Credit Card or Cash will be accepted).

_3. VOUCHER $ amounts:
___.... Full time employees........$79.41
___.... Part time employees.......$52.94
___.... PRN employees..............$26.47

_4. Employees are approved for the following: (Pharmacy and Rehab excluded)
___.... Full time employees........3 sets of uniforms
___.... Part time employees.......2 sets of uniforms
___.... PRN employees..............1 set of a uniform
___- you can achieve this by using RED ASTERISK * hospital preferred styles.

_**** NOTE discounted prices are only valid for Palms West employees and only for approved department colors .

_5. Any additional uniforms purchased will be employees responsibility.

_6. All special discounted prices are valid thru March 31st, 2014. The same special pricing will also be honored at MEDWEAR retail stores and the __onsite hospital sales at Palms West.

_7. MEDWEAR uniforms will be at Palms West hospital on the following Friday's:

__March 7th .......................1pm - 8pm ......... classroom #2 (LAST DAY FOR ONSITE SALES/PICKUP)

_Employees MUST be in compliance with the Uniform Color Requirements by April 2nd, 2014.

_8. Allow a minimum of 3 weeks for uniforms to be delivered if not in stock.

_9. ORIGINAL VOUCHER MUST be presented when ordering uniforms. If employee is pre-ordering online voucher must be provided at time of
___pickup.

10. NO RETURNS OR EXCHANGES if uniforms have been worn or washed.

11. NO RETURNS OR EXCHANGES on additional uniforms purchased after 30 days.

12. VOUCHER SALES ARE FINAL.

13. MARCH 31st IS THE DEADLINE TO REDEEM YOUR VOUCHER.

14. YOU CAN ONLY USE YOUR VOUCHER ONE TIME AND CANNOT RECEIVE A CREDIT IF YOU DO NOT USE THE ENTIRE AMOUNT.

15. VOUCHER CANNOT BE USED FOR SHOES, JACKETS, LONG SLEEVE T-SHIRTS OR ACCESSORIES.
